COLLAGE is a web and evidence based health and wellness assessement tool that promotes healthy aging and improves outcomes of older adults living independently.
COLLAGE is geared for those who are 55 or older and living independently in Northwest Philadelphia.
It offers individuals the opportunity to think about their hopes and goals as they age, explores ideas and strategies on how to achieve them and helps formulate a plan for making them a reality.
